Appointments
A 48 HOUR notice is required to change your appointment or a fee will be charged according to the amount of time booked for your reserved appointment. Failure to attend appointments or provide adequate notice deprives other patients of access to treatment (some of which may be urgent in nature) and adds significantly to the cost of everyone's dental care.
Dental Insurance
We require that the initial examination be paid by the patient and our office will submit the claim forms to your insurance company for a reimbursement to be mailed directly to you. The remaining treatment will be billed directly to your insurance company with the estimated patient portion to be paid at the time of service. Dental plans vary greatly. The forms, conditions, and percentages of payments are contracted between you, your employer, and the insurance company. the percentage of coverage relate to Insurance Company fee schedules, which may not necessarily correspond to the College of Dental Surgeons of BC fee schedule for Specialist.
Patient Responsibility
The cost of your dental treatment may be covered by a dental plan. If your dental plan does not cover 100% of the costs of your dental treatment, you are required to pay any outstanding balance. it is your responsibility to determin what percentage of the proposed treatment is covered by your dental plan. We routinely pre-authorize new and major treatment for our patients.
I have read and understood this statement and I agree that if my dental plan does not cover 100% of the costs of my dental treatment, i will pay the balance owing as previously arranged or when the services are rendered.
